Pet import requirements for Ukraine
What Ukraine asks for before a dog or cat can enter — the microchip, the rabies work, the permit and the paperwork — and how far ahead you need to start. We handle all of it on a move we arrange.

Where this fits in the timeline. The order matters more than the effort: a rabies vaccination given before the microchip was implanted does not count in most regimes, and a titer test drawn too early restarts a waiting period. Import requirements
12 weeks
Pets between 12 weeks and 16 weeks of age (only) that are either, 1.) not vaccinated against rabies, or 2.) 21 days has not elapsed since primary rabies vaccination, the pet must meet one of the following conditions (as noted on the model health certificate), the pet either has:
from birth until the time of the non-commercial movement the cats, dogs and ferrets have had no contact with wild animals of species susceptible to rabies, or their female parents have been vaccinated against rabies in accordance with OIE requirements before breeding.

Prior to shipping Vet import permit from the Governmental Department of Veterinary Medicine is required (this can take a few weeks to obtain) and guarantee that pet will proceed test in Ukraine.
Proforma-in voice is mandatory for pets shipped as manifested cargo
Health Certificate must be translated into Ukrainian or Russian. OR you may requesst to use a bilingual form which UA-veterinarian would be very much appreciated to get as an additional certificate to the International one from country of origin. Using this form will help to avoid problems and delays on import vet.control.
For unaccompanied pets: along with vet.documents simple proforma-in voice is required for import customs clearance The statement "value for customs purposes only"/"non commercial" must be mentioned in the proforma-in voice.
Please note in voice amount should not be too high as customs duties and VAT fees are based on total sum (in voice + freight cost)

Rabies and vaccinations
Must have valid anti-rabies vaccination (at least 30 days but not more than 12 months prior to shipment. Ukraine will accept a 3-year rabies vaccine, licensed for use in the United States. “Validity of vaccination” dates (i.e. 3-year rabies vaccine) must be completed. If the pet has only received one rabies vaccine (a primary vaccination), or if a rabies booster vaccine was administered outside of the period of validity of the previous vaccination, the pet will be required to wait at least 21 days after vaccination before travel to the Ukraine.
A neutralizing anti-body titration test
Dogs and Cats – not earlier than in 2 years and not later than in 3 months before the shipment the test for antibodies to rabies shall be carried out, at that the obtained positive result is with titer not less than 0. 5 immune unit per ml.
Animals shall be subject to the following veterinary processing and research:
Dogs – Toxoplasmosis;
Cats – Dermatophytosis
Health certificate and permits
Must have Health Certificate issued no more than 10 days prior to dispatch and stating that animal comes from a location free from contagious diseases of speciels in cluding Virus entsefalomielit, Disease Aueski, rabies, tubercoulosis, tulyaremiya, dermatofotisis (trihofitia, microsporia) for the
last 12 months + certified translation into Ukrainian or Russian. A health certificate issued by a licenced Veterinarian and endorsed by the exporting countries governing body.
